Absence Request Form

Generally speaking, we don not allow Bochurim to leave early, return late, or be absent during any normal Seder times. Please understand that absence is disruptive to learning, as well as to general running of the Yeshiva. However, we do understand that in some circumstances this will be needed. Please try to schedule any doctor appointments or other meetings during times when Yeshiva is off. If it is not possible at all, please use this form to request permission for absence.
As a safety and liability policy, no Talmid will be permitted to leave without written permission (using this form).
